Participant Forms
Because of the environmental conditions, the involvement of Southwestern Adventist University, Earth History Research Center, and the Hanson Research Station,
there are four one-page paper forms that each participant needs fill out, sign, and file with the project leaders before
any actual participation in the quarrying process is permitted.
All of the information supplied will be held in the strictest confidence.
All these forms are available when you arrive at the project site. However, you may wish to download them, fill them out, and bring the signed forms with you. The one form which may request information that you may not remember details is the SWAU and EHRC Medical Information Form.
In particular, you may find it convenient to download at least this one form and fill it out before leaving home!
If you are minor and not arriving with a parent or guardian, then you will need to download ALL four forms, fill them out, sign them, have a parent or guardian also sign them, and bring them with you.
